I, {contact_name}, on behalf of {name} (hereinafter referred to as “MINOR”) HEREBY WAIVE AND RELEASE, indemnify, hold harmless and forever discharge INLAND NORTHWEST GYMNASTICS and its agents, coaches, trustees, employees, officers, directors, affiliates, successors, volunteers and assigns, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I or MINOR ever had or may have, arising from or in any way related to MINOR’S participation in any of the events or activities conducted by, on the premises of, or for the benefit of, INLAND NORTHWEST GYMNASTICS provided that this waiver of liability does not apply to any acts of gross negligence, or intentional, willful or wanton misconduct. 

I understand that the activities that said MINOR will participate in are inherently dangerous and may cause serious or grievous injuries, including bodily injury, damage to personal property and/or death.  On behalf of myself, MINOR, my heirs, assigns and next of kin, I and said MINOR waive all claims for damages, injuries and death sustained to me or my property, that I or said MINOR may have against the aforementioned released party to such activity.

MINOR has the necessary and requisite skills to participate in all facets of, and activities of and requested of this facility, except as noted below. The nature of the activities has been fully disclosed and any flyer, advertisement, or brochure relating to the participating activities is expressly made a part of this WAIVER AND RELEASE.  I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately exercise my parental/guardianship rights and discontinue further participation by said MINOR in the activity.

By this Waiver, I, on behalf of said MINOR, assume any risk, and take full responsibility and waive any claims of personal injury, death or damage to personal property associated with INLAND NORTHWEST GYMNASTICS, including but not limited to using the facility and its equipment in any manner, form or fashion, and practicing and/or engaging in any aerobic activity or action sports activities, events or other related activities on and off the premises. 

This WAIVER AND RELEASE contains the entire agreement between the parties, and supercedes any prior written or oral agreements between them concerning the subject matter of this WAIVER AND RELEASE.  The provisions of this WAIVER AND RELEASE may be waived, altered, amended or repealed, in whole or in part, only upon the prior written consent of all parties.

The provision of this WAIVER AND RELEASE will continue in full force and effect even after the termination of the activities conducted by, on the premises of, or for the benefit of, INLAND NORTHWEST GYMNASTICS whether by agreement, by operation of law, or otherwise.                       

I have read, understand and fully agree to the terms of this WAIVER AND RELEASE.  I understand and confirm that by signing this WAIVER AND RELEASE said MINOR and I have given up considerable future legal rights. I have signed this Agreement freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to me.  My signature is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law. 

I, {contact_name}, as the parent or legal guardian of {first_name}, acknowledge that I have received and read the information provided by Inland Northwest Gymnastics Gymnastics regarding the risks associated with concussions, head injuries, and sudden cardiac arrest (SCA) in gymnastics activities. I understand and agree to the following:

1. Concussions and Head Injuries:

a. I understand that concussions are one of the most commonly reported injuries in sports and recreational activities, ranging from mild to severe.
b. I acknowledge that a concussion can occur in any organized or unorganized sport or recreational activity, including gymnastics, resulting from falls, collisions, or contact with surfaces or objects.
c. I understand that continuing to participate in gymnastics activities with a concussion or symptoms of a head injury can increase the risk of further injury, including catastrophic harm or death.
d. I agree to inform the appropriate coaches, staff, or officials if my child sustains a suspected concussion or head injury during gymnastics practice or competition.
e. I understand that my child will not be allowed to return to gymnastics activities until they have been evaluated by a licensed healthcare provider trained in concussion management and have received written clearance to resume participation.

2. Sudden Cardiac Arrest (SCA):

a. I understand that sudden cardiac arrest is the sudden onset of an abnormal and lethal heart rhythm, causing the heart to stop beating and the individual to collapse.
b. I acknowledge that SCA is the leading cause of death in the U.S., afflicting over 300,000 individuals per year, and it is the leading cause of sudden death in young athletes during sports.
c. I recognize that SCA in young athletes is usually caused by a structural or electrical disorder of the heart, which can be inherited (genetic) and may develop during adolescence or young adulthood.
d. I understand that SCA is more likely to occur during exercise or physical activity, placing student-athletes with undiagnosed heart conditions at greater risk.
e. I acknowledge that SCA can also occur from a direct blow to the chest by a firm projectile or by chest contact from another player (known as "commotio cordis").
f. I understand that some young athletes may have symptoms related to a heart condition but may neglect to tell an adult. If any of the following symptoms are present, a cardiac evaluation by a physician is recommended:

o Passing out during exercise
o Chest pain with exercise
o Excessive shortness of breath with exercise
o Palpitations (heart racing for no reason)
o Unexplained seizures
o A family member with early onset heart disease or sudden death from a heart condition before the age of 40.
